Tracey O’Shea: Okay, next question is “Why don’t you have your intake questionnaire forms before the initial consult? To me, it would seem like this would make more sense in order to have a better sense of what labs to get after the first appointment.”

So we do have an intake form prior to the [initial consult], and I will see about getting that over to you guys. It is something that I had made, actually, not too long ago because I was doing initial consultations and I also agree that I found that it was helpful. It’s pretty limited. It’s not as extensive as the intake for the case review. The intake for the initial consult primarily just says, like, what are your top five concerns, what medications are you on, what are your previous diagnoses, and then some family history. So I will see if I can get that over to you guys and give you some information on what that looks like. Again, it’s really brief; it’s really short. The majority of making [a] decision on what labs to do for the patient is determined when you’re talking to them during the initial consult, so for many, many years, we didn’t have an intake and it was just the responsibility of the practitioner who [was] doing the initial consult to gather that information to determine what labs to do in preparation for the case review. So I think it’s doable without the intake, but I think the intake does help with time management because then you kind of have a lot of that information right away and you’re not using some of that valuable 30-minute time for an initial consult just to kind of get information that you can already get in the intake form.
