Each Fellowship student will put together and present one one patient case study for discussion during a Grand Rounds live session. Use the form below to submit your case study information.
You will pick a patient from your Fellowship rounds that you observed an appointment with and gather the necessary information in their chart to fill out the case study submission form. Ideally, this be a patient that you can discuss within the live session group. Consider their primary complaint, the labs that were ordered, the results of those labs, and the recommended treatment plan from the practitioner, and bring any questions you have about the case or recommendations, etc.
The goal of the Grand Rounds presentation is to create discussion around the case amongst faculty and other students.
