Assist program participants in finding new ways to inspire improved health behaviors. Work closely with IHP customers and their members or employees in identifying ways to improve their chronic condition(s), promote healthy behaviors, and achieve their highest level of healthy functioning. Responsible for developing a health coaching relationship with participants and providing support, encouragement, and education. The Clinical Health Coach works in a team-based environment, partnering with nurses to provide support for clinical issues.
