Foundation for Medical Care is seeking a Spanish Speaking Health Coach who will be part of an interdisciplinary care team whose primary responsibilities are organizing and coordinating resources and services in response to individual healthcare needs along the illness and care continuum and in multiple settings. Assists with complex cases and internal process development.
