Responsible for: providing individual, supportive coaching to improve the physical health status of our patients; coordinating client care with the Certified Community Behavioral Health Clinic team, as well as with both internal/external providers; aiding clients in accessing/utilizing resources; and all other assigned tasks that promote the best outcomes for our clients.
