Health Coach

Weekly coaching based on motivational interviewing face-to-face (in person and virtually) or by phone if necessary.  Perform daily biometric measurements (weight, height, blood pressure readings).  Review participant evaluation reports and triage risk.  Lead group classes such as tobacco cessation and weight management.  Deliver “Health Breaks” or “Lunch-n-Learns” to the client.  Create and conduct quarterly competitions.  Work with clinic staff by identifying health risks that need further attention.
