Seeking bilingual health coach to be responsible for managing the care of patients who are identified by report, a referral from Case Manager or PCP and all patients discharged from hospital and Skilled Nursing Facilities. Work closely with Case Managers, PCPs, Medical Director and Office Manager to coordinate care of high-risk patients. Identify social determinants of health and address them such as food, transportation, housing, family support, etc.
