Responsible for engaging plan members in improved self-management and lifestyle behaviors for improved wellness and health management with a focus on members who have documented gaps in care. Facilitate engagement in the program, enhance members’ readiness to change daily lifestyle habits and adherence to evidence-based care guidelines resulting in improved overall wellness and improved clinical outcomes. Skillfully navigate dialog with members utilizing Motivation Interviewing to improve quality, reduce costs and enhance member experience. Awareness of when and how to access additional resources to meet needs of members.
