Accountable for providing transformation support to Community Health Centers (CHCs) resulting in improved quality outcomes, and network performance in CHPA’s value-based care (VBC) programs. Has a defined role, working in an advisory/coaching capacity to support CHPA CHCs and assist in creating and implementing individualize CHC CHPA Support Plans to optimize performance. Utilizes an evidence-based approach to facilitate QI methodology and tools within CHCs, including assessments, reporting measures, and facilitating meetings with practice specific QI teams working towards achieving measurable outcomes.
