Here are instructions and process for ordering Rifaximin from the CDD. There is some nuance and hurdles to the process.
- The patient orders directly from the CDD, following the instructions in the attached document below
- A MD will need to send a letter on practice letterhead (sample template below) as they do not accept prescriptions from FNP, PAs, etc.
- A prescription is written and then the patient sends over the printable (PDF) copy with the rest of their papwerwork.
- I generally write enough for the patient to have 4 weeks worth (or more if needed) because the cost of shipping and establishing care with them costs money. They come 56 in a bottle. So I tend to write this for 500 mg TID (these come in 500 mg instead of 550 mg) #112
Doctor’s note template on practice letterhead:
Dear CDD Pharmacy,
I am the treating physician for « name » and I have recommended a course of Rifaximin for the treatment of small intestinal bacterial overgrowth (SIBO).
