Do you recommend people do Paleo before [the] SIBO test or low FODMAP or high FODMAP?

Amy Nett: Liz asks, “Do you recommend people do Paleo before [the] SIBO test or low FODMAP or high FODMAP?”

Before the SIBO breath test, I mean, I would say generally around that time, I do want people to do a Paleo reset diet. I don’t think it matters too much when they start the Paleo reset relative to the SIBO breath test. I don’t do anything in terms of FODMAPs prior to the SIBO breath test because a low FODMAP diet is really a therapeutic diet. And so, if I’m going to use it, it’s going to be for a limited amount of time, specifically to address gut dysbiosis or gut-related pathologies. Otherwise, I mean, think about the foods that you’re missing out on in a FODMAP diet like onions, garlic, broccoli, cauliflower, Brussels sprouts—these things are super healthy, so I never want to do a low FODMAP diet unless I have a good reason, and then Liz is saying, “For the efficacy of testing, do you recommend anything?” Well, not really because I’m kind of curious with patients, like, if you’re having symptoms, ​what do you look like now? [36:18]​ So is what you’re eating in your diet, your lifestyle contributing to your symptoms right now? I actually don’t want to do a whole lot of changes to someone’s baseline before I do the testing. The only caveat is, again, because we get people with a lot of severe gut issues, and so, if I have someone who’s coming to the practice and they’ve been on a low FODMAP diet for the past year just trying to get by and manage their symptoms, sometimes, we probably will get in a way, like, a false-negative result. But again, the question is as you are, do you have small intestinal bacterial overgrowth? I probably wouldn’t recommend a therapeutic diet before the SIBO breath test because again I’m most interested in, as you are right now, what we are dealing with.
